Currently offering self pay only at this time. I would be more than happy to provide a superbill for you to submit to your insurance company for reimbursement.
Travel Fees: Additional $45 for consults greater than 20miles from Office.
Cancellations: I kindly ask that you provide a 24hr notice if you need to cancel or reschedule. Cancellation policy provided at the time of booking.
